دستور آپدیتم در کد زیر کار نمی کنه

سلام دوستان .من یک صفحه درست کردم که باید اطلاعات را تکمیل کنند.نمی دونم چرا دستور آپدیتم کار نمی کنی. آخه هیچ errori هم نمیده.
